PE-454609-1 MYOCARDIAL WORK IN VENTRICULAR PREEXCITATION

چکیده
Ventricular preexcitation alters the electromechanical activation sequence and may induce left ventricular (LV) contractile discoordination pathologic remodelling in selected cases. To evaluate LV myocardial work before after accessory pathway (AP) ablation patients with WPW to compare them normal controls. Mechanical mapping calculation was performed 11 pediatric syndrome/pattern sided or paraseptal APs undergoing catheter (successful 10/11) 8 healthy age-matched controls using speckle tracking echocardiography GE ECHOPAC version 204.57.0.745 software integrated AFI module. An 18-segmental echocardiographic model used. Global efficiency (GWE) calculated as ratio of global constructive sum wasted (GWW). The time from delta wave onset R peak lead V6 (delta R) used quantify degree preexcitation. Before GWW higher (164 vs 102 mmHg%, P=0.015) GWE lower (median 91 94%, P=0.010) After successful decreased 164 93 mmHg% (P=0.006) increased 91.1 94.0 % (P=0.006). tended correlate negatively interval (R=0.59, P=0.056). ablation, segmental WE distance AP insertion median 69.5 at location 98.0 distant segments (P<0.001). equally distributed among (Figure). localized a (IQR) 0 (0-1) EP mapped location. induces significant inefficiency adjacent which correlates is normalized ablation. amount asymptomatic be considered when assessing potential for discussing indication prophylactic localize manifest APs.

Asymptomatic Ventricular Preexcitation
Background—Sudden cardiac death can be the first clinical presentation of asymptomatic ventricular preexcitation. Methods and Results—From 1995 to 2005, we prospectively collected clinical and electrophysiological data among 293 adults with asymptomatic ventricular preexcitation (61.4% males; median age, 36 years; interquartile range [IQR], 28 to 47.5). After electrophysiological testing, patie...
متن کاملAlternating Ventricular Preexcitation
A 45-year-old woman diagnosed with Wolff-Parkinson-White syndrome preferred treatment with propafenone rather than catheter ablation. A 12-lead electrocardiogram (Fig. 1) after the drug administration showed sinus arrhythmia; alternating ventricular preexcitation; and alternating T wave inversion in the inferior leads during normal AV conduction. Ventricular preexcitation may disappear because ...
متن کاملUnmasking of Ventricular Preexcitation
Twenty-one patients were studied in whom ventricular preexcitation (VP) had been recorded in the past and had later disappeared, indicating antegrade block in the accessory pathway (AP), either spontaneously (10 patients) or under the effect of chronic treatment with amiodarone (11 patients). VP reappeared in nine cases during vagal stimulation, and in five cases during an i.v. isoproterenol in...
متن کامل[Treatment of patients with ventricular preexcitation].
Ventricular preexcitation, due to an accessory pathway that creates an electrical connection between the atria with the ventricles, can provide a substrate for a variety of arrhythmogenic syndromes, from paroxysmal AV junctional tachycardia, to the facilitation or aggravation of atrial tachyarrhythmias that can lead to sudden death. However, statistics show that the most frequent situation is f...
متن کاملVentricular preexcitation modulates strain and attenuates cardiac remodeling in a swine model of myocardial infarction.
BACKGROUND Myocardial infarction modifies the distribution of stress within the heart, increasing wall stress in ischemic and surrounding tissue, which often leads to adverse left ventricular remodeling. Electrical preexcitation pacing with appropriate timing of high-stress regions can reduce local strain and may attenuate global remodeling. METHODS AND RESULTS Myocardial infarction was induc...
